JOB TITLE:    Home Care Assistant II 
DESCRIPTOR:    Assisting  persons with in-home services
REPORTS TO:    Homemaker Supervisor           

PURPOSE OF POSITION: To help clients  maintain independence through homecare & personal assistance

1.    Observation of client functioning and identifying and reporting changes to  his/her supervisor or employer;     
2.    Following client’s written Plan of Care, including assistance with household chores, including cooking and meal preparation, cleaning and laundry;
3.    Assistance in completing activities such as shopping and appointments outside of the home and providing necessary receipts and documentation in the conduct of essential shoppong/errands;    
4.    Completion of appropriate records documenting daily activities, observations, and direct hours of service; and      
5.    Assistance with activities of daily living and assistance with personal care as required
6.    Attending pre-service training, in-service training sessions, and staff conferences.

1.    Responsibilities as identified by the regulatory systems as identified in the addendum
2.    Assistance with medication reminders      
3.    Office assistance as needed, to be as flexible as possible with clients needs in mind.
4.    Other duties as assigned

1. A high school diploma or general education diploma; or
2. One year employment in a comparable human service capacity, or experience in care for a dependent child or adult family member; or
3. Demonstration of continued progress towards meeting the
educational requirement of a general education diploma by current registration and evidence of successful completion of course work (successful completion means achievement of a grade of "C" or higher);and
4. Has had previous documented and supervised training within the past 2 years prior to this employment, equivalent to 24 hours of homecare aide pre-service  training, as determined by the provider with appropriate documentation in the employee's personnel file; or
5. Has successfully completed RN, LPN, MD, physician assistant or CNA training in the past and has been employed in the field within the past 2 years; or
6. Has been employed as a CCP homecare aide within the past year; or
7. Any current HCA completing LSSI designated specialized training as verified by their supervisor and documented  in personnel file
